1. About This Privacy Policy
This Privacy Policy explains how Integrity Wellness collects, uses, stores, and protects information when you use our website, contact us, request materials, create an account, purchase a program, or use member features.
2. Information We Collect
Depending on how you use Integrity Wellness, we may collect information such as:
- your name and email address;
- information you provide when contacting us;
- your date of birth;
- answers to eligibility questions;
- food selections and preferences;
- information about your eating habits, activity, routine, schedule, and goals;
- questionnaire responses;
- your personalized plan;
- check-in and progress information;
- program and purchase information; and
- records showing acceptance of applicable agreements or purchase acknowledgments.
Integrity Wellness does not ask members to provide food-allergy information as part of its personalization process. You remain responsible for avoiding foods or ingredients that are unsafe or inappropriate for you.
3. How Your Information Is Stored
Some information is stored securely through service providers that help us operate Integrity Wellness.
Account information, questionnaire responses, personalized plans, progress information, purchase records, and applicable agreement or acknowledgment records may be stored server-side through our member database provider.
Eligibility screening responses are currently processed and stored locally in your browser and are not stored in the Integrity Wellness member database.
We may also use browser storage or strictly necessary session technologies to keep you signed in and provide website and member functionality.
4. How We Use Information
We may use information to:
- determine program eligibility;
- create and provide your personalized nutrition plan and member resources;
- provide check-ins, progress features, and customer support;
- create and manage your account;
- process and document purchases;
- maintain records of agreements and acknowledgments;
- respond to questions or requests;
- deliver materials you request; and
- operate, secure, and improve our services.
We do not use health information to diagnose, treat, or manage medical conditions.
5. Consumer Health Data
Certain information requested during the Integrity Wellness eligibility process may be considered consumer health data under applicable law.
This may include information about pregnancy or breastfeeding, certain serious medical conditions or treatments, recent major surgery, kidney disease requiring dietary management, an active eating disorder, GLP-1 medication use, or whether a physician or registered dietitian has instructed you to follow a medically prescribed or therapeutic diet.
We use this information only to determine whether an Integrity Wellness nutrition program may be appropriate for you.
Current eligibility screening information is processed and stored locally in your browser. It is not stored in the Integrity Wellness member database.
Integrity Wellness does not sell consumer health data.
We do not use advertising pixels or cross-site behavioral tracking to collect consumer health data.
6. Contact Forms, Email & Requested Materials
When you contact Integrity Wellness, we receive the information you choose to provide so we can respond to you.
Website forms may be processed through Vercel and delivered through Google Workspace/Gmail.
If you request the Favorite Foods Makeover Collection or another free resource, we may use the name and email address you provide to deliver the requested material. Requesting a resource does not, by itself, enroll you in an ongoing marketing email list.
7. Purchases & Payment Information
Payments may be processed by PayPal or another payment provider identified at checkout.
Integrity Wellness may retain purchase information such as the program or product purchased, amount, currency, payment status, transaction or order identifier, and purchase date.
Integrity Wellness does not receive or store your full credit-card or bank-account information. Payment credentials are handled by the payment provider.
8. Agreements & Purchase Records
When required, Integrity Wellness may maintain records showing that you accepted a Participation Agreement, acknowledged what was included in your purchase, or completed other required purchase confirmations.
These records may include your account identifier, the applicable agreement or disclosure version, acceptance date and time, and a link to the applicable purchase record.
Integrity Wellness does not intentionally collect or retain your IP address or browser User-Agent as evidence of these acceptances.
9. Analytics, Cookies & Browser Storage
Integrity Wellness does not currently use website analytics platforms, advertising pixels, or behavioral advertising trackers.
We may use browser storage, cookies, or similar strictly necessary technologies for account authentication, security, session management, and website or member functionality.
We do not currently use these technologies for behavioral advertising.
10. Service Providers
We use service providers that help us operate Integrity Wellness. These may include:
- Vercel for website hosting and server functions;
- Supabase for authentication and member database services;
- PayPal for payment processing;
- Google Workspace/Gmail for email and communications; and
- Google Fonts, whose externally hosted font services may receive standard browser request information such as IP address and User-Agent.
These providers may process information as necessary to perform services for Integrity Wellness and may maintain ordinary operational, security, or transaction records under their own practices.
We do not sell your personal information.
11. Photos & Testimonials
If progress-photo functionality is available, photos you choose to provide are used for member progress features and are not permission to use those photos for advertising or marketing.
Integrity Wellness will not publicly use your progress photos, testimonial, or success-story materials without separate permission.
12. Your Privacy Rights
Depending on where you live and applicable law, you may have rights concerning personal information Integrity Wellness maintains about you, including rights to request access, correction, or deletion.
Where consumer health data laws apply, you may also have rights to:
- confirm whether Integrity Wellness collects, shares, or sells consumer health data relating to you;
- identify applicable third parties with whom that information has been shared or sold;
- withdraw consent where applicable; or
- request deletion of applicable consumer health data.
Eligibility information that remains only in your browser may be removed by clearing the applicable browser or site storage. Doing so may affect website functionality.
To make a privacy request, contact info@integritywellness.us. We may need to verify your identity before completing a request.
We will respond to qualifying requests within the time required by applicable law.
13. Appeals
If Integrity Wellness declines to act on a consumer health data request and applicable law gives you a right to appeal, contact info@integritywellness.us and state that your message is a Privacy Request Appeal.
We will respond within the period required by applicable law and provide any additional information required by law.
14. Data Retention & Deletion
We retain information for as long as reasonably necessary to provide our services, maintain appropriate business and legal records, document purchases and agreements, resolve disputes, prevent fraud or misuse, enforce our agreements, and comply with applicable law.
Information stored only in your browser generally remains there until it is cleared, overwritten, or otherwise removed.
Where applicable law requires deletion of information maintained by Integrity Wellness, we will process qualifying requests in accordance with applicable law.
15. Security
Integrity Wellness takes reasonable measures appropriate to the nature of the information we maintain to protect it from unauthorized access, use, or disclosure.
No electronic transmission or storage system can be guaranteed to be completely secure.
16. Children
Integrity Wellness nutrition programs are available only to adults age 18 and older.
Individuals under 18 are not eligible to participate.
17. Changes to This Privacy Policy
We may update this Privacy Policy when our practices, technology, service providers, or legal obligations change.
If applicable law requires notice or consent before a material change involving consumer health data, we will provide it as required.
The current effective date will appear at the top of this policy.
18. International Visitors
Integrity Wellness is operated from Nevada, United States.
If you access our services from outside the United States, laws in your jurisdiction may provide additional privacy rights. Nothing in this Privacy Policy limits rights that cannot legally be waived.
